Observation

staff was not aware of what the children were doing at all times, didn't know that a child from the group was not present but instead in the bathroom. staff did not consider ages, layout of space or ages when supervising children going to the bathroom. staff has since changed bathroom supervision procedures to include one child at a time to use the bathroom, writing name on whiteboard outside of bathroom to signal in use and staff member standing outside watching the door. ??
